"It's Okay to Not Be Okay" Episode 46 leaves the audience caught in confusion wondering who the real Ingrid is—Olivia (Maricel Laxa), Eden (Agot Isidro), or Zelda (Ana Abad-Santos), the new player in this mind game.
The episode opens with Mia (Anne Curtis) entering the mansion, her thoughts slipping back to a childhood birthday when her mother Ingrid (Empress Schuck) surprised her with a chocolate cake and a knife. Samuel (Geoff Eigenmann) could only watch helplessly as Ingrid allowed young Mia to wound herself with the blade.
The scene cuts to Eden stopping by a bakeshop to order a cake with a birthday dedication, which she says is for her child. It follows Olivia next, walking poised in her black dress as she leaves OK Hospital. Doc Emman (Mark Oblea) compliments Eden, who only smiles when teased about probably meeting a special someone.
Another series of flashback follows, revealing why Dipong holds such devotion to the promise of Ingrid’s return. On that night, Ingrid emerged from the lake bloodied, her white dress stained and her face covered with wounds, with Dipong as the only witness. As he moved closer to help, a furious Ingrid clawed his face with her long nails, which explains his scar.
Dipong took her into his home so she could change clothes and tend to her injuries. He suggested calling the police, but Ingrid refused. All she said was that she would come back for her daughter Mia and take revenge on her husband. She firmly instructed Dipong to remain silent and tell no one what had happened, and he obeyed through the years. The desire in his eyes would explain why. But the next morning, Ingrid was gone. The flashbacks further reveal that Ingrid sought the help of an uncle (Epi Quizon) who was a surgeon, as if only to have the wounds on her face treated. Yet she broke down in hysteria when she looked into the mirror, screaming that the face staring back at her was not her own.
At OK Hospital, an emergency throws Patrick (Joshua Garcia) and Gemma (Kaori Oinuma) into panic when they find Pogi (Aljon Mendoza) unconscious on the ground outside, with a syringe and vial beside him suggesting he has been sedated. Moments later, Pogi wakes and recounts how Zelda attacked him, telling him that this day is special to her. Piecing the details together, Patrick borrows Gemma's car and speeds to Mia's mansion.
Patrick’s suspicion proves right—Zelda has gone to Mia’s house. Dressed in black and humming the eerie lullaby that triggered Samuel at the facility the other day, Zelda brings with her a birthday cake for Mia, whom she addresses as “anak.” Mia opens the doors to her but refuses to believe Zelda’s claims of being her mother, convinced that the woman is nothing more than a fan.
Suspense escalates when Zelda instructs Mia to get a knife from the kitchen, ostensibly to slice the cake. Mia studies the knife's sharp blade with a grim stare before she finds Zelda in the library, going through the shelves. Fury flashes in Mia’s eyes as she tells Zelda to leave but the latter only seems amused. Zelda's voice grows darker, her eyes heavy and unsettling, as she recalls Samuel nearly killing her and calling them both monsters.
Tension spikes as Zelda picks up a pen kite from Mia's desk, taunting her about the sorrow of having a father who tried to kill her on her birthday, as though she knows more about Mia’s past than she should. Mia snatches the knife by the blade, only for Zelda to wrench it free slowly, slicing Mia’s palm.
“Why are you doing this?” Mia demands, her eyes burning with anger. Zelda answers with mocking calm, “Why not?”
Could Zelda be Ingrid? Just outside the mansion, Dipong keeps watch and waits for “Ingrid,” whose return he has long been anticipating.
