While recuperating in isolation at the facility, Matmat (Carlo Aquino) came across a strange note, while Patrick (Joshua Garcia) continued to drown—not in the river, but in the painful memories of the past haunting him in Episode 41 of “It’s Okay to Not Be Okay.”
Still refusing to face people after his outburst and confrontation with Patrick in front of the patients and staff of OK Hospital, Matmat ignored anyone who entered his room. But he couldn’t resist when Jason (Francis Magundayao) personally brought him chicken inasal, sure of how to win him over. As soon as Jason left, Matmat opened the takeout boxes, which also included soup from Nanay Li (Rio Locsin), and murmured a heartfelt thanks to the two of them for the meal they had prepared.
Before he could even take a bite, another knock came at the door, which he assumed was Jason but only to find no one outside—just a book lying on the floor. It was titled "Ang Pagpatay sa Mahiwagang Bruha" and at the back was a chilling note that read, “Pinatay ng bunsong kapatid ang manong niya.”
Eden (Agot Isidro) suddenly appeared peeking at the door of Matmat’s room, and the scene cut to Zelda (Ana Abad-Santos) acting strangely as she admired Matmat’s painting in the mess hall, while Olivia (Maricel Laxa) watched her patient with growing concern.
Meanwhile, Mia (Anne Curtis) appeared at OK Hospital determined to make Patrick talk to her. She repeatedly told him that he had no fault in everything that had happened, that he should stop being held hostage by the past and by her family because she herself was there when it all happened. She witnessed how Matmat accidentally fell into the river, and she even admitted that back then she wished Patrick would let him drown, but Patrick was only a little boy who loved his brother too much to stand by, so he found a way to save him, nearly drowning himself in the process if not for Mia rushing in to pull him out.
Mia wanted Patrick to understand that he needed her as the key to freeing him from all his chains. But Patrick insisted that he's old enough to know better—that in his desire to be with Mia, he ended up hurting Matmat, which he could no longer forgive himself for. "Mali na in-entertain kita sa buhay ko," he told her, his voice trembling as he struggled to hold back the tears he had long mastered hiding.
He finally cut her off from his life, dismissing what they once shared as nothing more than a short-lived dream. Mia returned to the mansion crying as she lay on Patrick’s bed longing for his presence, while Patrick chose instead to be by his brother’s side, holding Matmat’s hand.
As he kept his eyes on his brother, Patrick was once again carried back to the painful memory of when they were just children and he uttered that cruel wish for Matmat to die so that he would no longer be burdened with taking care of him, followed by the haunting image of their near-drowning in the river, and then every struggle they had endured together through the years that only strengthened the bond of their brotherhood, all those moments when Patrick did nothing else but look after Matmat, defend him with all he had, and sacrifice everything in his life just for his 'manong.'
