Still searching for answers, Patrick (Joshua Garcia) finds the mysterious and unpublished final manuscript of Ingrid (Empress Schuck) titled “Ang Anino sa Likod ng Bagong Mukha: Ikalimang Libro sa Buhay ng Bruha.” To his shock, the pages only confirm his fear that Mia (Anne Curtis)’s mother, Ingrid, was Tess (Meryll Soriano)’s murderer.
Building on Matmat’s earlier revelation about their mother’s killer, Episode 54 of "It's Okay to Not Be Okay" opens with Patrick convincing himself again and again that it can’t possibly be Mia’s mother who ruined his life. Yet the evidence points straight at her. The closeness he now shares with Mia, which she even calls destiny, suddenly feels like fate’s cruel way of leading him toward the truth.
Patrick then questions Dipong, the caretaker of the Hernandez mansion who once worked alongside his mother. But Dipong insists he knows nothing, since it was Samuel (Geoff Eigenman) he often spent time with back then. Even so, Patrick keeps his eyes on the mansion from the outside and whispers to himself that only within those walls lies the truth he has been seeking.
Meanwhile, the series continues to play with viewers' curiosity through the smallest details, like the mystery of who Dipong calls to inform that Patrick has been asking questions. The scene then cuts to Eden on her phone, before she checks on Samuel (Michael De Mesa) and tells him, “Nandito ako hanggang sa huling hininga mo."
Of Samuel's declining health condition, the nurses at OK Hospital are weighed down by sadness as he seems to merely await the hour of his death, or, as he describes it, the conclusion of his punishment on earth. When Gemma (Kaori Oinuma) spots Mia inside the facility visiting Matmat, she urges her to also visit her father and try to understand his situation. But Mia refuses, insisting she has never felt that Samuel was truly her family.
A flashback shows a young Mia (Ysabella Suarez) being poisoned by her mother’s words, told that because Ingrid created her, she alone has the power to destroy her. So when Samuel (Geoff Eigenmann) arrived home, Mia interrogated him in a tone that echoed Ingrid until he snapped and grabbed her by the neck, as though he would rather see his daughter dead.
Meanwhile, back in the mansion, Patrick resumes his hunt for clues, beginning with Ingrid’s butterfly brooch pin which he believes is the same one Matmat saw on their mom's killer. His search eventually leads him to the library, where an unusual wooden book-shaped box sits atop the shelves. Curious, Patrick opens it and is stunned to find Ingrid’s final and unpublished manuscript titled “Ang Anino sa Likod ng Bagong Mukha: Ikalimang Libro sa Buhay ng Bruha.” To recall, this is the same manuscript Zelda (Ana Abad-Santos) had tried to find when she entered the mansion after escaping the facility.
Patrick reads through the manuscript. "Para sa bruha, walang puwang sa mundo ang mga mabababang uri ng tao, mga aliping nag-aakalang may halaga ang kanilang mga boses. Sabi ng bruha, ‘Kahit ako'y mawala o mailibing, hindi makakatakas ang aliping mapanghimasok. Ang aliping dumarating tuwing bukang-liwayway ay mabubura sa mundo gamit ang aking mga kamay.’
The passage unsettles Patrick, his thoughts spiraling. Did the witch kill the servant? Could this “servant” be his mother? And did Ingrid write this passage as a chilling account of what truly happened back then?
"Madaling mawala sa landas ang aliping mas madumi pa sa putik. Mapangahas ang alipin na may dalawang supling, parehong ipinanganak na lalaki." Patrick anxiously turns the next page. "Ang nakatatandang supling ay inutil, may kulang sa pag-iisip. Habang ang isa naman, bagama't normal ay gaya ng kanyang ina na putik at mapangahas. Sa kagustuhang kaibiganin ang pinaka-iniingatang pag-aari ng bruha, ang nag-iisang dalagitang prinsesa."
"At nagtagumpay ang bruha. Wala na sa mundo ang alipin. Isinakatuparan niya ang pangalawang hakbang ng malagim na plano, ang pagbabago ng anyo at pagpapalit ng pagkatao. Habang pinapanood na mamatay sa dusa ang dating katuwang sa buhay." As Patrick reads the part of the story about the witch’s transformation, the scene cuts to Eden (Agot Isidro) and Olivia (Maricel Laxa), both tense as they wait for Doc Anok (Bobot Mortiz)'s announcement on who between them will be promoted as Nurse Manager.
Mia begins to wonder why Patrick seems to be avoiding her again when he dismisses her idea of taking a family photo with Matmat, and then heads straight to Matmat’s room instead to tell him how proud he is of his courage. Even later that night, Mia is still caught up in her thoughts about what could be running through Patrick’s mind. Even Jason (Francis Magundayao), who personally delivers her chicken inasal order, ends up in a friendly chat with her as she probes what he knows about Patrick, to which he replies that Patrick is someone who’s mastered concealing his emotions and problems behind his nonchalant façade. By the way, Francis and Anne share a nice rapport in this scene, which likely comes from how naturally talented they are as actors.
While Mia has no clue, Patrick consults a lawyer about the possibility of reopening his mother’s murder case now that he has uncovered new details which make him certain of who the killer is—Ingrid Hernandez.
